Here are what forms you will need:

• Every adult patient and every guardian of a minor patient needs a Notice of Privacy Practices.The federal government requires that you read this form, but you need not return it to us.
• After reading this form, you will need to read and sign Acknowledgement of Privacy Practices.
• Every patient needs a Patient Registration.
• Every adult needs a Medical History.
• Every child needs a Child History.
• Each insured adult needs an Insurance Agreement.

Once completed, don’t forget to bring the forms to your appointment!
